15-year-old EHR system blocking innovation and creating risk
Cloud-native platform rebuild with HIPAA-compliant architecture
The Challenge
The organization's 15-year-old electronic health record system was blocking every digital initiative — too slow to support modern clinical workflows, too brittle to integrate with new tools, and increasingly difficult to keep HIPAA-compliant. A prior modernization attempt had failed after 18 months and $12M spent. Leadership needed a partner who could both design and execute.
Our Approach
Paxanimi conducted a thorough architecture assessment before writing a single line of code. We designed a cloud-native replacement built on modern standards (HL7 FHIR, SMART on FHIR) with security and HIPAA compliance architected in — not bolted on. We used a phased migration approach that kept the legacy system running throughout, eliminating cutover risk. Our security practice ran HIPAA compliance tracking in parallel with engineering delivery.
The Result
The new platform launched on schedule across all facilities. Performance improved by 3× on every measured workflow. HITRUST certification was achieved four months after go-live. The old system was fully decommissioned six weeks later. The organization has since expanded the engagement to cover two acquired facilities using the same architecture.
“We came to Paxanimi for a technology assessment. We left with a transformation roadmap our board approved unanimously. That never happens.”
Key Outcomes
- Platform performance improved 3× end-to-end
- HIPAA compliance and HITRUST certification achieved
- 15-year-old EHR system fully decommissioned
- 99.97% uptime since go-live — zero patient data incidents
- Clinician satisfaction scores increased 40% post-launch
